The Respondent Prize Table is Now $5000 for 2016!
This is a dramatic increase from the $2000 available in previous years! Each practicing dentist survey respondent now has chance to win one of a $2500 Cash Grand Prize; Three $500 Runner Up Prizes; or Ten Consolation prizes of $100 each*.

Also new for 2016, the survey will only be available on-line as requested by many respondents – the previous print version has been discontinued.
